Role Description Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Registered Nurse for a PRN field position serving patients throughoutForney/Terrell/Mesquite and surrounding communities. This role involves visiting patients in their homes to provide skilled nursing care, including assessments, medication administration, wound care, and monitoring of symptoms and vital signs. The nurse will develop and implement individualized care plans, educate patients and caregivers on disease management and home care techniques, and coordinate services with physicians, therapists, and other care team members. Daily responsibilities include accurate documentation, communication with the interdisciplinary team, and timely reporting of changes in patient condition. Qualifications Strong clinical Nursing skills, with current licensure as a Registered Nurse (RN) in the state of Texas. Foundational knowledge in Medicine to support safe care, clinical decision-making, and collaboration with physicians. Experience in Home Care, including managing care in varied home environments and promoting patient independence. Proficient in direct Patient Care, including assessments, medication administration, symptom management, and family education. Competence in Wound Care, Wound Vac, IV including dressing changes, infection prevention, and monitoring healing progress. Excellent communication, interpersonal, and documentation skills, with the ability to build trust with patients and families. Reliable transportation and ability to travel locally to patient homes; comfort working independently in the field. Prior experience in home health, palliative care, or hospice is preferred; basic computer and EMR skills are beneficial. OASIS and WellSky/Kinnser experience is a plus #J-18808-Ljbffr
