Title Nurse Practitioner - Advanced Practice Provider Company Oak Street Health Location 1625 Rio Bravo Blvd SW Suite 36, Albuquerque, NM 87105 Overview Oak Street Health is a mission-driven primary care provider focused on delivering personalized, value-based care to older adults on Medicare. As a Nurse Practitioner at Oak Street Health, you will provide effective, equitable, value‑based primary care, delivering longitudinal care and building meaningful patient relationships while working on a collaborative care team that includes physicians, nurse practitioners, physician assistants, medical assistants, and support staff. Core Responsibilities Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments with a holistic focus. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, ensuring eligible patients receive evidence‑based screenings. Assist the care team with phone triage and outreach. Educate patients on health conditions, care plans, and treatments. Participate in organizational promotional activities. Conduct home visits as needed. Complete other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in at least one specialty: Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner. Active, non‑probationary state Nurse Practitioner license. Active DEA license. US work authorization. Demonstrated embodiment of “Oaky” values. Preferred Qualifications Experience in primary care: internal medicine, geriatrics, or family medicine. Familiarity with HCC documentation, ICD‑10 coding, and health risk assessments. Passion for teamwork and cross‑functional collaboration. Desire to be part of an empirical, population‑health‑focused model. Bilingual proficiency in applicable areas.
