Occupational Therapist (OT) - Home Health

Position Overview: The Home Care Occupational Therapist evaluates patients of all ages, develops and updates treatment plans, and collaborates with the healthcare team to improve or restore daily functioning. They serve as a clinical resource, educate patients and families, and ensure documentation meets regulatory standards. Key Responsibilities Review and communicate care plans with physicians, document thoroughly, and update goals as patient conditions change. Educate patients and caregivers on therapy techniques, discharge planning, and home safety assessments. Order supplies, manage schedules, and coordinate outpatient and community resources. Supervise and instruct Occupational Therapy Assistants and Home Health Aides, ensuring compliance with home care regulations. Complete comprehensive evaluations, assessments, and documentation, including OASIS forms and home safety recommendations. Observe, report patient reactions, and adapt therapy plans accordingly. Qualifications And Skills Licensed Occupational Therapist with BLS certification within 30 days. Valid driver's license and proof of auto insurance. Bachelor's Degree in Occupational Therapy or related field. Strong documentation, communication, critical thinking, and patient education skills.
