Bilingual Care Manager /Social Worker (English/Spanish) - Broward/Miami-Dade (On-Site)

Job Description
The Bilingual Care Manager is responsible for coordinating care, advocating for clients, and supporting families through complex medical and social service systems. This role works closely with healthcare providers, legal professionals, and community resources to ensure clients receive comprehensive and high-quality care.

Key Responsibilities:

• Coordinate care plans for elderly and special needs clients
• Assist clients and families with Medicaid and long-term care applications
• Communicate and collaborate with physicians, therapists, and care teams
• Prepare documentation and reports to support care planning and legal processes
• Advocate for clients and ensure access to appropriate community resources
• Develop and implement individualized, client-centered care strategies
• Monitor client progress and adjust care plans as needed

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.

Requirements

Qualifications:

Required:

• Bilingual in English and Spanish
• Experience in home care, healthcare case management, or social services
• Strong communication, organization, and care coordination skills
• Ability to work independently with strong critical thinking and follow-through
• Bachelor's degree required

Preferred:

• Degree in Social Work or related field
• Experience with Medicaid, Social Security programs, or long-term care systems
• Knowledge of care coordination for complex populations
• 701B Certification (preferred)
