Primary Care Physician

Role: Primary Care Physician (PCP) responsible for providing comprehensive care to older adult patients with acute and chronic conditions. Key Responsibilities Independently assess, diagnose, treat, and manage patient health, including health promotion and care coordination. Take full accountability for patient outcomes, consulting specialists when needed, and engaging with hospitalists for hospitalized patients. Lead a care team comprising medical assistants, care facilitators, and coordinators, ensuring quality and continuity of care. Manage care for patients across various settings, including in‑office, hospital, SNF, LTC, or homebound, collaborating with transitional care teams and other resources. Participate in center management, recruitment, and coverage responsibilities. Qualifications & Skills MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar; board eligibility required, certification preferred. Active State Medical License; DEA license; BLS certification within 90 days. Proficient in evidence‑based medicine, lifestyle, and nutrition factors impacting health. Strong leadership, communication, and influence skills; dedicated to high‑quality patient care. Proficient in Microsoft Office; willing to travel locally, regionally, and nationally as needed.
