Care Coordinator (Care Coordinator II)

Department Overview This is a clinical role that is vital in supporting the work of the nurse coordinators, physicians and Advanced Practice Providers within the Knight Cancer Institute by independently performing a variety of clinical administrative and technical functions. The Care Coordinator performs system-focused activities that ensure care is seamless across providers and the care continuum. Care coordination bridges gaps between facilities, specialists, labs, community resources, and primary care. The position coordinates and manages all non‑nurse patient caseloads, working with providers on clinic days to provide clinical support with order entry, appointment coordination, and order management for a specialized patient population. The incumbent reviews patient records with the provider to coordinate care needs during clinic visits and throughout the cancer treatment episode, using independent judgment while following direction from nurse coordinators and physicians. The role builds and maintains working relationships with internal institutional departments that provide services to Knight Cancer Institute patients. Function / Duties of Position Patient management – Responsible for coordinating and managing the non‑nurse aspects of the patient's clinical care process. Reviews the plan of care, provides support with order entry, AVS preparation, medication refills, and education to patients and families. Places lab and scheduling orders and follows up with patients. Monitors the Epic inbox for messages, refills requests, and manages appropriate actions. Provides glucose meter use education and performs authorizations and refill requests. Care coordination – Ensures patients follow through with outside referrals to other specialties, labs, and diagnostic imaging. Coordinates hospital admission and cell collection with inpatient and ancillary services for bone marrow transplant patients and donors. Communication – Provides program information and education to patients, families, and referring physician office staff. Participates in multidisciplinary patient conferences and serves as the communication link among physicians, referral offices, inpatient teams, and the Ambulatory Oncology clinic. Responds to voicemail, email, MyChart, phone, and fax inquiries within priority guidelines and typically within a two‑day turnaround. Attends MD/department team meetings as appropriate and triages patient questions. Documentation / Data Management – Documents legibly, concisely, and completely to enable assessment of patients' progress. Uses established checklists and sign‑out procedures, participates in database input, monitors active tracking tools, and provides real‑time updates. Initiates and implements processes for new protocols and tracking tools. Program coordination – Arranges satellite clinics, works with TC and RNC to ensure patient results and orders are completed and updated in Epic, attends weekly huddles, and orders and schedules ancillary services and procedures. Other tasks and duties as assigned. Required Qualifications Three years’ experience in a hospital or hospital‑based clinic setting. High School diploma or equivalent. Positions with in-person patient care: BLS certificate within 30 days of hire or prior to independent practice, whichever comes first. One of the following three: Completion of a nationally recognized accredited medical assistant training program, including a practicum of at least 160 hours. Successful completion of a formal medical services training program of the United States Armed Forces. Current Oregon Emergency Medical Technician (EMT) license (basic or advanced) and national EMT registration with the National Registry of Emergency Medical Technicians (NREMT). Current Oregon Practical Nurse license. For dental school hires only: Completion of a Dental Assistant training program and successful completion of the DANB exam. For those completing medical assistant training or formal military medical services training, an MA certification from a nationally recognized and accredited certifying body must be received upon hire or by the end of the probationary period, including: The American Association of Medical Assistants (AAMA), awarding the Certified Medical Assistant (CMA). The American Medical Technologists (AMT), awarding the Registered Medical Assistant (RMA). The National Center for Competency Testing, awarding the National Certified MA (NCMA). The National Health Career Association, awarding the Certified Clinical Medical Assistant (CCMA). Must maintain applicable certifications or licensures for the duration of employment. Demonstrated ability to work independently and as a member of a team, including leadership. Knowledge of patient flow and back‑office functions; ability to perform chart scrubbing and panel outreach activities efficiently. Strong analytic skills, proficiency with EPIC and Microsoft Office (Excel, Word, PowerPoint). Time‑management skills, ability to meet competing deadlines, and strong customer‑service orientation. Working knowledge of medical terminology and ability to build relationships with patients, including those with special needs. Preferred Qualifications Experience with Epic and Cadence (scheduling module). Additional Details Position works in an extremely fast‑paced environment with many interruptions, competing priorities, and frequent interruptions. Interacts with people at all levels of the organization and external customers. Work schedule: five days a week, 8 hours per day, with potential longer hours for biweekly patient review meetings. Position requires ability to sit, stand, walk quickly throughout the hospital campus, and use heavy phone, PC, and EMR systems. Requires respectful, caring, and non‑judgmental communication.
