Unit Details

The Occupational Therapy unit in a Skilled Nursing Facility provides inpatient rehab for patients who need daily skilled care but not hospital-level treatment. Its goal is to help residents regain independence, improve quality of life, and safely transition home or to another setting. OTs assess physical, cognitive, and psychosocial abilities and create care plans focused on daily activities such as bathing, dressing, grooming, toileting, mobility, and household tasks. They address fall prevention, cognitive and memory issues, and train patients in adaptive equipment and home modifications. OT staff collaborate closely with the healthcare team on discharge planning. Most patients are older adults recovering from illness, injury, or surgery; stays range from short-term rehab to long-term care. Typical therapy is 6090 minutes a day, five days a weekless intensive than inpatient rehab.

The OT process follows evaluation, intervention, and re-evaluation tailored to each patient. Upon admission, the OT performs a comprehensive assessment of function, goals, home setup, and support systems, then designs a treatment plan. Interventions include ADL training (bathing, dressing, grooming, toileting, eating), functional mobility and transfers, IADL training when appropriate, and therapeutic activities to improve strength, motion, and cognition. OTs also educate patients and families on safety and adaptive equipment. They document progress and skilled need (including MDS requirements) for coordination and reimbursement. Working with the interdisciplinary team, OTs plan discharge to home or the least restrictive setting.
