Description

• Assess patients in both inpatient and outpatient environments and develop customized treatment plans
• Implement therapeutic interventions focusing on mobility, strength, balance, and overall functionality
• Collaborate smoothly with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Educate patients and their families about therapeutic goals, discharge processes, and home exercise regimens
• Maintain thorough documentation of evaluations, treatments, and progress in line with facility rules and regulations
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Additional certifications (e.g., in neurological, geriatric therapy) are advantageous but not essential

Skills

• Previous experience is highly preferred
• Capability to address the needs of patients with complicated medical and mobility challenges
• Strong communication skills and ability to work collaboratively in a team
