Hospice Registered Nurses collaborate to provide management and delivery of patient care plans. This position supports patients in Plum, PA, and surrounding areas. Eligible for a $10,000 first‑year success bonus. Responsibilities Collaborate with care teams to manage and deliver patient care plans. Provide direct nursing care to hospice patients and their families. Maintain timely and accurate charting to meet critical deadlines. Self‑manage time and schedule to meet patient needs. Utilize tablets, mobile phones, and EMR software effectively. Travel up to 20% of the day to patient locations. Qualifications RN license in the state of employment. Associate degree or higher from an accredited School of Nursing. Two years of RN experience; hospice experience preferred. Current driver’s license. Commitment to meet critical charting deadlines.
