Pediatric Clinical Pharmacist

Schedule Details / Additional Information The Pediatrics Pharmacist provides patient-centered care to the pediatric patient population. This position must provide pharmaceutical care to patients to achieve optimal therapeutic outcomes; promote cost-effective drug therapy; provide assistance in educating pharmacy students, PGY1 and PGY2 residents; adhere to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building; assist with verification of medication orders and distribution of pharmaceutical products; coordinate and participate in the education of pharmacists and other health care professionals; participate in drug therapy management in emergency situations; assist in developing and implementing treatment protocols to improve patient care. Schedule: 1st, 2nd and weekend rotations. Major Responsibilities Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. Use effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. Design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens based on relevant biomedical literature; implement recommendations effectively using policies and procedures and document in the appropriate format. Review and accurately interpret medication orders and prescriptions. Participate in management of medical emergencies when applicable. Actively support department medication safety and quality improvement activities, including event reporting through appropriate channels. Perform quality assurance and performance improvement of the medication use system as appropriate. Collaborate with the healthcare team to manage formulary adherence and the medication-use process, and assist in the acquisition of non-formulary, compassionate use, and investigational medications. Practice in compliance with organization’s policies and procedures, as well as local, state, and federal regulations. Prepare, dispense, and oversee the proper storage and handling of medications in accordance with best practice, policies, and state and federal regulations. Serve as preceptor for pharmacy students, residents, and healthcare learners as assigned. Mentor, orient, train, and assist with the performance evaluation of pharmacy teammates, residents, and students as assigned. Direct and check the accuracy of the work of pharmacy support team members in accordance with the organization’s policies and procedures. Licensure, Registration, and/or Certification Required Licensed in the state(s) of practice or eligible for reciprocity. Immunization and non-vaccine injectable certification required based on area of practice. Credentialing and privileging required based on area of practice. Education Required Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP. Experience Required No experience required. PGY1 and/or PGY2 Pediatric residency experience or previous children’s hospital experience preferred. Knowledge, Skills & Abilities Required Maintains skills in area of practice. Strong computer skills. Excellent communication and organization skills. Excellent presentation, facilitation, and training development skills. Demonstrates decision making, team building, customer service, and conflict resolution skills.
