Ensure safe, evidence-based interventions aligned with organizational policies and clinical standards. Implement and monitor individualized patient care plans. Evaluate outcomes and adjust treatment to optimize patient safety and recovery. Administer physician and nursing orders promptly. Maintain compliance with regulatory standards. Advance quality improvement initiatives. Lead discharge planning efforts. Collaborate with patients and the interdisciplinary team for safe transitions. Facilitate effective post-acute care planning. Work primarily with a cardiac surgery and advanced heart failure patient population. Handle high-risk cardiac surgeries, including open heart recovery, heart transplants, LVAD implants, and ECMO.
