**Job Summary:**

**Responsibilities:**

* Serve as a primary point of contact for clients, responding to inquiries, and addressing concerns in a timely and professional manner
* Collaborate with care teams to develop personalized care plans that meet each client's unique needs and goals
* Facilitate effective communication among clients, caregivers, and families to ensure a cohesive care experience
* Maintain accurate records and documentation of client interactions and care plans
* Identify and escalate any concerns or issues that may impact the quality of care
* Provide education and support to clients and their families on home care services and available resources
* Participate in quality improvement initiatives to enhance client satisfaction and care outcomes

**Requirements:**

* **Client Liaison** experience in the healthcare industry, preferably in home care or hospice
* Excellent communication, interpersonal, and problem-solving skills
* Ability to maintain confidentiality and handle sensitive information
* Strong organizational and time management skills, with attention to detail
* Familiarity with electronic medical records and documentation systems
* High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred
