Physical Therapist - Home Health Location Gassaway, WV Assignment Duration This is a 13-week contract position. Schedule 8-hour shifts, Monday through Friday, 8 am to 4:30 pm. Job Description We are seeking a dedicated Physical Therapist to provide quality care to patients in a home health setting. You will treat a range of conditions including general orthopedics, generalized weakness, strengthening exercises, and cardiac rehabilitation. Responsibilities Evaluate and develop treatment plans for patients. Implement therapy interventions to improve patient mobility and functionality. Document patient progress in the EPIC charting system. Collaborate with a team of healthcare professionals to ensure comprehensive care. Manage a caseload of 5-6 patients per day, including admissions and discharges. Required Experience / Certifications / Licensure Current Physical Therapist license for the state of West Virginia.
