Help Launch a New PACE Program
This separate business entity helps older adults with complex medical and social needs remain safe, independent, and connected to their communities.
The organization is seeking a mission-driven Geriatric Services Manager to lead and help build the social work function for its new PACE center. This is a unique opportunity to join before the first participant is enrolled and help shape the care model, team culture, workflows, and participant support structure from the ground up.
This is not a therapist role, and the organization is not requiring an LCSW for this position. The ideal candidate is a strong social work leader with an MSW, a deep commitment to older adults, and the ability to work effectively in a team-based care environment.
The Geriatric Services Manager will report to the Center Director and work closely with the interdisciplinary team, PACE leadership, and community partners. The role will build and oversee the social work team while supporting high-quality psychosocial care, participant advocacy, care planning, and family support.
Social Work Leadership and Program Launch
Partner with the Center Director and PACE leadership to prepare the geriatric services and social work function for launch.
Help develop social work workflows, documentation standards, staffing plans, team expectations, and participant-support processes.
Build, supervise, mentor, and support the social work team.
Provide leadership, coaching, accountability, and day-to-day guidance to Geriatric Services Specialists and related team members.
Promote a strong culture of compassion, teamwork, participant-centered care, and service to older adults.
Support the development of a high-functioning interdisciplinary care model.
Participate in program development, quality improvement, regulatory readiness, and team training.
Help establish a strong social work practice environment, including partnership opportunities connected to UC Berkeley.
Participant Assessment and Care Planning
Lead and participate in psychosocial assessments, care planning, reassessments, and service coordination for PACE participants.
Partner with the interdisciplinary team to identify participants' social, emotional, behavioral, family, cultural, environmental, and community-resource needs.
Develop and support individualized care-plan interventions that help participants remain safe, connected, and supported.
Monitor participant changes and communicate timely updates to the interdisciplinary team.
Support participants and caregivers through aging-related needs, dementia, grief and loss, family dynamics, caregiver stress, housing concerns, financial barriers, and care transitions.
Geriatric Services and Care Coordination
Serve as a key advocate for older adults and their families within the PACE model.
Coordinate with participants, families, caregivers, community agencies, hospitals, skilled nursing facilities, contracted providers, and other service partners.
Support home visits, hospital visits, skilled nursing facility visits, discharge planning, and post-discharge follow-up as needed.
Help participants and families access appropriate community resources, benefits, services, and supports.
Provide education and guidance to participants, caregivers, and staff regarding aging, psychosocial needs, dementia support, end-of-life considerations, and the PACE care model.
Quality, Documentation, and Compliance
Maintain timely, accurate, and complete social work documentation in the electronic health record.
Support compliance with PACE requirements, organizational standards, documentation expectations, and participant-care protocols.
Participate in quality-improvement activities, audits, process improvements, staff education, and regulatory-readiness efforts.
Support a consistent, high-quality, culturally responsive approach to social work services across home, center, hospital, skilled nursing facility, and community settings.
Required Qualifications
Master's degree in Social Work required.
Clinical Licensure as an LCSW, ASW, etc., is not required.
At least two years of health-related social work, care management, geriatric services, or related experience.
Experience working with older adults, frail seniors, medically complex populations, or vulnerable communities.
Prior leadership, supervision, team-building, or program-development experience.
Strong understanding of psychosocial assessment, care coordination, participant advocacy, community resources, and documentation standards.
Ability to work effectively in an interdisciplinary, team-based care environment.
Deep interest in, or commitment to, serving older adults through a coordinated care model.
Ability to complete local travel to participant homes, hospitals, skilled nursing facilities, and community settings as needed.
Preferred Background
PACE, geriatric care, community health, home-based care, social services, case management, managed care, Medicare Advantage, or managed Medicaid experience.
Experience building or supervising a social work team.
Experience supporting frail seniors, participants with cognitive decline, complex family dynamics, caregiver stress, grief and loss, housing instability, or social barriers to care.
Bilingual ability, especially Cantonese, Mandarin, or another Asian language, is strongly preferred.
Ideal Candidate
The strongest candidate will be a social work leader who genuinely cares about older adults and wants to help build a meaningful program from the ground up.
This person should be comfortable creating structure, building trust across teams, leading staff, partnering closely with clinical and operational leaders, and working within a highly collaborative interdisciplinary care model.
The right candidate does not need to be a therapist. They need to be a strong social work leader who understands geriatric care, participant advocacy, family support, care coordination, and the importance of culture fit in a new PACE program.
Compensation and Benefits
Estimated base salary of $100,000-$120,000, based on experience and qualifications.
