About This Role The LTSS Service Care Coordinator assists in developing, assessing, and coordinating holistic care management activities for long-term care members, focusing on quality and cost-effective healthcare outcomes. This role is ideal for candidates with case management or home visit experience. Highlights Pay: $22.94 - $38.79/hour Location: Murphysboro, US Schedule: Full Time, Day shift Residency: Applicants should reside within Jackson County, IL Work Style: Combines remote work with approximately 50% local travel for in-home member visits What You'll Do Evaluate the needs of the member and recommend/facilitate plans for the best outcome Assist with developing ongoing long-term care plans/service plans Coordinate between the member and care provider team to ensure services are accessible Provide resource support to members and their families/caregivers Monitor care plans/service plans and provide recommendations based on member needs Interact with long-term care healthcare providers to ensure member needs are met Collect, document, and maintain long-term care member information Perform on-site visits to assess member's needs Provide education to long-term care members and their families/caregivers Provide feedback to leadership on opportunities to improve care quality Requirements Education: Bachelor's degree required Experience: 1 year of related experience required Licenses & Certifications: One of the following licenses required: RN licensed in Illinois, OR Bachelor or Master's Degree in a human services related field, OR LPN with one (1) year experience in conducting comprehensive assessments for the elderly Benefits Competitive pay Health insurance 401K and stock purchase plans Tuition reimbursement Paid time off plus holidays Flexible work schedules About Centene Centene is a leading healthcare provider committed to delivering exceptional patient care.
