In-Home Occupational Therapist (Contract) — Immediate Start

Jukebox Health Inc.

Jukebox Health Inc. is seeking aContracted In-Home Occupational Therapist to support older adults and individuals with disabilities in Ramapo, NY. This role focuses on in-home assessments, personalized recommendations for safe home modifications, and fall-prevention guidance. The OT will work with a remote clinical team, maintain documentation in our electronic system, and coordinate care with clients and families.
