Supportive Housing Case Manager Up to 1500 Retention Bonus

Qualifications :

To Qualify Applicants must meet one of the criteria listed below:

• Bachelors Degree in social services or related field and a minimum of one year experience in the coordination of supportive housing or in the coordination of independent living services in a social service setting   or  
• A High School Diploma or GED and two years of experience in the coordination of supportive housing or in the coordination of independent living services in a social service setting.

Ideal Candidates will also meet the following qualifications:

• Self-starter excellent time management skills and proactive problem solver.
• Ability to work a flexible schedule.
• Technology savvy especially with Microsoft Office Applications Zoom PC and Android devices.
• A valid drivers license insurance and access to a vehicle insured for you as a driver are required
• Ability to perform CPR/First Aid as needed.
• Ability to lift up to 50 lbs.
• Must be at least 21 years of age.
• Bilingual Preferred
• Good communication is key in this role. Youll rely on your reading writing and communication skills to document care follow support plans and collaborate with both internal and external team members.
