Responsibilities:

• Assess patients' cognitive, physical, and sensory skills to develop effective therapy plans

• Implement and modify therapy plans to address patients' specific needs and goals

• Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal outcomes

• Monitor and document patients' progress, adjusting therapy plans as needed

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination

• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
