Home Health Registered Nurse

As our Home Health RN you will be the clinician families trust to turn complex needs into safer, more independent living—reducing hospital returns and restoring quality of life across Utah communities.

What You Will Accomplish

• Complete comprehensive in-home assessments and create individualized care plans that achieve 90%+ patient-centered goal attainment within 30 days of start of care.
• Deliver skilled nursing visits and family education that measurably improve self-management and reduce unplanned hospitalizations by at least 15% for your caseload.
• Coordinate real-time with physicians and the interdisciplinary team so every care transition is seamless and 100% of visits are documented accurately in the EMR the same day.
• Identify and resolve barriers to care (transportation, supplies, social needs) so patients remain safely at home and survey-ready compliance is maintained on every chart.
• Mentor new clinicians and contribute to quality reviews that keep Rocky Mountain Care’s home-health outcomes among the region’s strongest.

Environment & Resources

You report to the Clinical Director and work alongside a tight-knit team of nurses, therapists, and social workers. You will use a modern EMR, receive mileage reimbursement or a company vehicle, and have dedicated scheduling and intake support so you can focus on patients. Territory is primarily Davis and surrounding counties with flexible daily routing.

Essential Qualifications

• Current Utah RN license and BLS; home-health or comparable community-nursing experience that has already produced measurable patient-outcome improvements.
• Proven ability to complete OASIS-accurate assessments and write care plans that drive functional gains.
• Comfort educating patients and families under time pressure while maintaining 100% documentation compliance.
• Valid driver’s license and reliable transportation for independent home visits.
• Strong clinical judgment that keeps patients out of the hospital and survey-ready.
