Housing Case Manager

Urban Peak

Urban Peak in Denver, CO is seeking a Housing Case Manager to provide supervised youth support, case management, and crisis counseling across shelter and housing settings. You will help youth develop service plans, coordinate with partner agencies, and document services in HMIS. The role emphasizes trauma-informed care, safety, confidentiality, and collaboration with interdisciplinary teams.
