Responsibilities: Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities Monitor and report changes in patient condition to the nursing team and physicians Collaborate with other healthcare professionals to develop and implement individualized care plans Demonstrate a strong understanding of infection control, fall prevention, and other safety protocols Participate in facility-wide quality improvement initiatives to ensure the highest level of care About the Role: As a Licensed Practical Nurse at Eagle Pointe Skilled Nursing, you will have the opportunity to make a meaningful difference in the lives of our residents.
