Registered Nurse Care Manager

Location 204 South Bellevue Avenue,Dover, OH, 44622,United States Employee Type Full Time Non Exempt Manage Others No SpringVale Health Centers, Inc. is seeking a Registered Nurse for a Care Manager position to provide a variety of nursing duties in a Medical office setting. This is a full-time position based in our Dover medical office. Key Responsibilities: Conduct initial and periodic holistic assessments of patients, including physical and psychological evaluations. Maintain ongoing telehealth relationships with patients/members in a Care Management relationship. Develop and implement individualized care plans based on patient assessments. Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care delivery. Facilitate patient access to community resources and support services to enhance care and promote independence. Participate in quality improvement initiatives to enhance care delivery and patient outcomes. Maintain contracted payer relationships. Assist Nursing Coordinator with coverage of duties requiring an RN in their absence. Minimum of three (3) years of professional nursing experience, preferably in care management, case management, or a related field. FQHC experience and knowledge of HRSA, CARF, CPC, and VBC are highly preferred.
