Albany, NYDescriptionGENERAL RESPONSIBILITIES:The physician (MD/DO) functions in the primary care group providing direct patient care, and clinical oversight related to the assigned location/program. The provider works closely with the Chief Medical Officer, Community Prevention and Treatment Services (CPTS) Program Director and others to review and ensure that the performance of the program is in accordance with the institution’s Performance Improvement Safety plan goals. This role includes the dissemination of best practices, protocols, and training to enhance staff skills and knowledge in the prevention and treatment of HIV and HCV infections. SPECIFIC RESPONSIBILITIES: Leadership Responsibilities:Serves as clinical champion for Community Prevention and Treatment Services program (CPTS) for NCQA PCMH principles.Leads program clinical quality improvement activities and serves as SME for the Quality Improvement CommitteeSupports established policies and procedures of the Health Center and makes recommendations and contributes to policy formulation.Makes recommendations and contributes to the development of clinical protocols and best practice guidelines.Education and Mentorship:Provides ongoing education and mentorship to medical providers. Contributes to medical staff professional development and adherence to best practices aiming to ensure patient safety and optimize treatment outcomes. Includes chart reviews, documentation, providing one-on-one mentorship and on-site supervision, and group presentation as required.Functions as a consultant and preceptor, to the primary care physicians and non-physician practitioners as appropriate, Supervises the clinical practice of medical students, residents, non-physician practitioners as appropriate and contributes to the learning of students from other disciplines.Acts as a resource to staff and the community regarding current trends in health care as well as specific health issues.Participates in community activities and research designed to promote health and prevent diseases.Clinical Responsibilities:Provides primary care directly, emphasizing accessibility, health promotion, illness prevention and continuity of care to patients presenting with minor and complex health issues.Takes a complete history including medical, psychosocial, economic, relationship and family status.Completes physical examinations, performs/orders appropriate diagnostic tests.Discusses health concerns/diagnosis with the patient and together determines an appropriate plan of care.Develops, reviews, and approves clinical protocols and best practice guidelines.Quality of Service:Documents all patient encounters completely and in a timely manner. This includes proper documentation of phone calls, prescription, and the dictation of patient visits for a session prior to leaving the health center.Adheres to clinical practice guidelines that are adopted by the specialty group or an appropriate committee.Keeps medical knowledge current through attendance at medical CME programs at least 40 hours per year.Maintains an acceptable level of compliance with clinical and regulatory standards including medical record documentation in accordance with Joint Commission standards.Demonstrates appropriate interactions toward patients in a manner that helps to put patients at ease and adheres to organizational values in interpersonal interactions. Participant in the Performance Improvement activities of the Health Center through chart audits, development and implementation of practice standards.Actively supports the implementation of the Patient Centered Medical Home model of care at all clinical sites.Conducts care in a manner that meets NYS AIDS Institute and HRSA HIV/AIDS Bureau clinical guidelines for care of HIV/Hep C patients and those at risk.Teamwork:Works collaboratively with the multidisciplinary team to provide treatment, ongoing follow-up, and evaluation of the plan of care with the patient.Supports established policies and procedures of the Health Center. Makes recommendations and contributes to policy formulation.Starts patient schedule on time and leaves after completing the work schedule including needed record documentation.Promptly completes required documentation for referrals and consultations.Returns patient telephone calls within an appropriate time frame based on patient needs and expectations.Shares in after-hours call coverage with other clinicians as required.Performs other administrative tasks as assigned by the Chief Medical Officer.Miscellaneous:Demonstrates excellence in both internal and external customer service.Understands and can effectively communicate HIPAA compliance, corporate compliance, and client confidentiality.Ensures and/or remains in compliance with local, state, and federal regulation, i.e., DHHS HRSA and NYSDOH, and all accreditation standards (e.g., Joint Commission and NCQA-PCMH).Adheres to the National Patient Safety Goals as defined by the Joint Commission on Accreditation of Healthcare Organizations and Whitney M. Young Jr. Health Services.Completes other duties as assigned.RequirementsMINIMUM QUALIFICATIONS:Graduation from an approved medical college and completion of approved residency training. Current NYS license and DEA. Board certified or eligible MD/DO in Internal Medicine, Family Practice, or Infectious Disease. PREFERRED QUALIFICATIONS:Two years of clinical experience preferred. HIV Specialist Certification. Experience in Community Health. Bilingual/Multi-lingual abilities. Experience in delivering culturally competent healthcare.
