Position Personal Service Coordinator/ASW MFT APCC – 35 hours per week / $34.00–$34.50 per hour (actual compensation based on experience and legal factors). Benefits Clinical supervision hours Coverage for required trainings and seminars, including 3‑day DMH-sponsored Dialectical Behavior Therapy training Paid time off for training and licensing exam preparation Performance Pay bonus program with potential to earn an additional $1,600 per month Summary Under the general supervision of the program manager, assist individuals in accessing services through intensive case management. Provide mental and/or physical health services as needed, perform assessments, and prepare documentation required by funding sources following the psychosocial rehabilitation model. Essential Duties and Responsibilities Provide coaching, counseling, mentoring, training, and positive reinforcement for members in areas such as grooming, hygiene, coping skills, public transportation, and independent living skills. Perform intakes, initial assessments, and other evaluations required by DMH contracts. Conduct outreach and engagement services for FSP referrals and Mini Mental Status Exams. Develop Recovery Plans or Personal Service Plans. Assist clients in locating and obtaining services including housing, food, health care, substance abuse treatment, and benefits. Apply harm reduction philosophy and motivational interviewing techniques. Advocate for clients and mediate service access. Engage with members in community settings. Provide individual and group therapy. Participate in staff and community meetings and support day‑to‑day operations. Maintain documentation standards in Electronic Health Records (EHR) and CareLogic System, ensuring timely billing and compliance. Position Requirements Master’s degree in Social Work (MSW) or Counseling (MA/MS) and registration with the California Board of Behavioral Sciences as ASW/AMFT/APCC Clinical experience in mental health, disability, and/or underserved populations preferred Willingness to become Licensed Professional Social Worker (LPS) for 5150 holds Participation in program’s supervised hours to pursue Licensed Clinical Social Worker status Valid Class C driver’s license, vehicle, insurance, and liability insurance clearance Empathetic approach and ability to navigate barriers for individuals in need Knowledge of community resources, housing, social security, and relief systems Team‑orientated and fast‑paced work style Proficiency in Microsoft Word, Excel, and Outlook Valid CPR certificate preferred Flexible schedule, including evenings, weekends, and after‑hours on‑call rotations TB clearance and annual re‑testing COVID‑19 vaccination policy compliance (policy may evolve) Employment verification from three past employers within the last seven years #J-18808-Ljbffr Mental Health America of Los Angeles
