While Working Here, You Will Enjoy Employee Engagement Activities Positive work environment Excellent Training Competitive Pay and Benefits Job Description Assist the Social Services Director in identifying and providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge. Responsibilities Of The Social Worker Assist in collecting social history data and in the development of a comprehensive discharge plan, including the MDS and Care Plan for new admits, as directed by the Social Services Director. Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes. Assist families with the engraving and/or marking of resident’s personal items. Document progress notes, which relate to each resident’s care plan when necessary and within policy timeframe. Requirements/Qualifications Of The Social Worker At least one (1) year experience in social work, preferably in a long-term care facility. Effective verbal and written English communication skills. Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
