Description

• Conduct evaluations of patients in inpatient or outpatient settings and create personalized treatment plans
• Carry out physical therapy interventions focused on improving mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
• Inform patients and their families about therapeutic objectives, discharge plans, and home exercise programs
• Document evaluations, therapies, and progress in compliance with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure a coordinated approach to care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., in neurology or geriatrics) are an asset, though not mandatory

Skills

• Previous experience is strongly preferred
• Ability to treat patients with complex medical and mobility issues
• Excellent communication and teamwork skills
• Comfort with digital charting and clinical documentation systems
