Key Responsibilities
- Evaluate and treat patients in a skilled nursing facility (SNF) and subacute rehabilitation setting
- Develop and implement individualized physical therapy treatment plans for geriatric patients
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Educate patients and caregivers on therapeutic exercises and home programs
- Active Physical Therapist (PT) license in the state of Florida
- Graduate of an accredited Physical Therapy program
- Current BLS/CPR certification
- Ability to work effectively in a SNF or subacute rehabilitation environment
- Prior experience in a SNF or subacute rehabilitation setting
- Experience working with geriatric patient populations
- Opportunity to make a meaningful impact on the recovery and quality of life of geriatric patients
- Supportive contract placement with a dedicated staffing team to assist throughout your assignment
