Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This opportunity seeks a licensed Pharmacist with strong expertise in Medication Therapy Management (MTM) to support specialty MTM services. The position is a full-time, 13-week contract to hire role, with the possibility of transitioning to a permanent position. Candidates may work onsite or remotely but must hold a current Louisiana pharmacist license and at least five additional licenses from a specified list of states.

Key Responsibilities:

• Accurately prepare and dispense drug orders based on physician requests following established protocols.
• Interpret verbal and written prescriptions and update computerized patient medication profiles.
• Maintain complete and accurate pharmacy patient profiles and dispense pharmaceuticals safely.
• Manage controlled substances and keep meticulous records in compliance with legal requirements.
• Monitor drug therapy regimens for interactions, contraindications, allergies, and appropriate dosages.
• Detect and promptly report adverse drug reactions.
• Provide clinical consultations and recommend cost-effective therapeutic alternatives to medical staff.
• Document all clinical interventions thoroughly.
• Participate in departmental quality improvement and medication use reviews, inspections, and recordkeeping.
• Supervise pharmacy support personnel and verify their daily activities.
• Organize and prioritize work to ensure timely pharmacy service delivery with minimal supervision.
• Conduct medication storage inspections and manage expired stock.
• Provide professional customer service by answering phone calls and assisting at the pharmacy window.

Qualifications:

• Current Louisiana pharmacist license required.
• Must hold at least five licenses from the following states: DC, North Dakota, New Hampshire, New Jersey, South Carolina, South Dakota, Utah, Wyoming — with an expectation to expand to a total of 10 additional states within 18 months.
• Prior experience in retail or hospital pharmacy settings.
• Strong knowledge of medication therapy management principles.
• Ability to perform independent and supervisory duties effectively.
