Registered Nurse, Care Coordinator - General Internal Medicine

Responsibilities Collaborate with clinic team members to provide evidence-based patient-centered care Help patients stay up to date on care plans and goals Focus on high-risk medically complex patients, patients on disease registries, and care transitions Act as a liaison with patient’s family members and other agencies Facilitate improved communication for comprehensive whole person care Stay up to date on standards, assisting with programs and processes Requirements Associate Degree in Nursing from a CCNE or ACEN accredited nursing program Current Ohio Registered Nurse license CPR/BLS certified Knowledge of triage procedures Clinic, physician office and/or urgent care experience Ability to maintain quality, safety, and/or infection control standards Knowledge of appropriate procedures and standards for administration of medications and patient care aids Ability to educate and clearly communicate with patients and/or families regarding disease nature and proper care and treatment High level of interpersonal skills to handle sensitive and confidential situations Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions #J-18808-Ljbffr Jobtailor
