Registered Nurse, Care Coordinator - Post Covid Care

Responsibilities Collaborates with clinic team members to provide evidence-based patient-centered care Helps patients stay up to date on care plans and related goals Focuses on high-risk medically complex patients, including those in care transitions Acts as a liaison with patients’ family members and other agencies Facilitates improved communication to provide comprehensive whole-person care Assists with programs and processes, staying up to date on standards Requirements Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program Current Ohio Registered Nurse license CPR/BLS certified Knowledge of triage procedures along with clinic, physician office and/or urgent care experience Ability to maintain quality, safety, and/or infection control standards Knowledge of appropriate procedures and standards for the administration of medications and patient care aids Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment High level of interpersonal skills to handle sensitive and confidential situations Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions Core Competencies Demonstrates expertise in evidence-based patient-centered care, focusing on high-risk medically complex patients.
