Registered Nurse, Care Coordinator - Campus Area

Responsibilities Collaborates with clinic team members to provide evidence-based patient-centered care Helps patients stay up to date on care plans and related goals towards improving health Focuses on high-risk medically complex patients, patients on specific disease registries, and patients in care transitions Acts as a liaison with patient’s family members and other agencies Facilitates improved communication with the aim of providing comprehensive whole person care Stays up to date on standards, assisting with programs and processes Requirements Associate Degree in Nursing from a CCNE or ACEN accredited program Current Ohio Registered Nurse license CPR/BLS certified Knowledge of triage procedures along with clinic, physician office and/or urgent care experience Ability to maintain quality, safety, and/or infection control standards Knowledge of appropriate procedures and standards for the administration of medications and patient care aids Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment High level of interpersonal skills to handle sensitive and confidential situations Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions #J-18808-Ljbffr Jobtailor
