Remote Physician Collaborator for Senior Care

Humana Inc. is seeking a Physician Collaborator for CenterWell Senior Primary Care. This fully remote role reports to the Associate Director and supports APPs who provide wraparound services for patients with recent fractures who cannot see their PCP. You will close gaps in care and coordinate monthly chart reviews to ensure quality patient care. Travel to Humana offices may be required for training or meetings. #J-18808-Ljbffr
