Occupational Therapist - Registry/Per Diem

Performs Occupational therapy evaluation, reassessment and consultation; provides plan of care, discharge planning, patient, and significant other education; renders occupational therapy services within the practice guidelines of the American Association of Occupational Therapy (AOTA) and other regulatory agencies.

Essential Duties and Responsibilities:

• Performs Occupational Therapy evaluation, reassessment and consultation upon referral of a physician, dentist or podiatrist.
• Provides plan of care and discharge planning. Secures a physician certificate for the plan of care.
• Provides Occupational Therapy services within the practice guidelines of the AOTA, the Illinois Occupational Therapy Practice Act and other regulatory agencies.
• Participates in discharge planning, when requested.
• Communicates with patient's physician, nurse and case manager when necessary regarding the patient's evaluation report, progress and discharge plan.
• Ensures patient safety, patient rights and HIPAA compliance.

Qualifications:

• Current IL Occupational Therapy licensure
• BLS Certification
