Outreach Development
Intake Support Specialist (PT)
The Intake Support Specialist provides administrative and follow-up support to the Centralized Intake team to ensure timely access to care, reduced no-show rates, and accurate referral tracking.
Essential Duties:
- Conduct appointment reminder calls, texts, and follow-up outreach.
- Conduct client re-engagement outreach for individuals who miss appointments or disengage from services.
- Conduct client satisfaction surveys and document outcomes.
- Follow up on missing intake documentation and required forms.
- Maintain waitlists, referral trackers, and outreach logs.
- Assist with referral follow-up and outcome reporting.
Minimum Requirements:
- High school diploma or equivalent.
- Strong communication, customer service, organizational, and multitasking skills.
- Computer proficiency and ability to learn and utilize electronic medical record systems.
Preferred Qualifications:
- Bilingual.
- Experience in behavioral health, substance use treatment, healthcare intake, care coordination, customer service, or call center operations.
Position Status:
This is a part-time position.
Monday - Thursday: 12pm -8pm Friday: 4pm - 7pm 19 hours per week.
