Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Travel: ~20% local travel to patient homes

Overview

Join our client’s dedicated care team as a Home Health Physical Therapy Assistant , delivering compassionate, patient‑centered support directly in the home setting. Key Responsibilities

• Collaborate with the clinical team to support the management and delivery of patient care plans
  
• Complete charting and documentation within required deadlines
  
• Self‑manage daily schedule, travel, and patient visits effectively
  
• Maintain a high standard of professionalism, communication, and patient engagement
  

Travel Expectations

• Ability to spend approximately 20% of the workday driving to and from patient homes

• Monday – Friday, 8:00 AM – 5:00 PM
  
• No weekends or holidays
  

Requirements

Required Qualifications

• Associate degree or higher
  
• 1+ year of experience in ANY of the following:
  
• Home Health Care
  
• Home Health
  
• 1+ year of experience in ANY of the following:
  
• Electronic Medical Record (EMR) systems
  
• Practice Partner EMR
  
• EHR Implementation
  
• Valid Driver’s License (PA)
  
• Active Physical Therapy Assistant (PTA) License in Illinois (IL)
  
• Has not applied to or worked for Residential Healthcare Group within the last 6 months
  
• Comfortable with required travel for the position
