• Current and valid LPN license

• Proficient with EMR documentation and following detailed care plans
• Current CPR certification

Schedule/Hours:

• Schedule based on patient assignments and visits

Key Responsibilities:

• Deliver direct nursing care and support to patients in their homes, following physician and interdisciplinary team instructions
• Monitor vital signs, administer medications, and provide wound care according to ordered plans
• Educate patients and families about ongoing health management and necessary lifestyle adjustments
• Maintain detailed and accurate patient records, ensuring compliance with healthcare regulations
• Collaborate with physicians, therapists, and other home health team members to optimize patient health outcomes
