Bilingual RN Care Planner: Home-Based Senior Advocate
SCAN Group
SCAN Group is seeking a field/remote based RN care manager for Los Angeles County to provide long-term care management to frail, economically disadvantaged older adults and caregivers. You will assess eligibility, certify nursing home needs, and develop in-home care plans with a focus on staying at home and avoiding premature institutionalization.
