Integrated Care Case Manager for IDD & Behavioral Health

A community health organization in Texas is seeking a Collaborative Care Case Manager to manage a caseload of individuals with intellectual and developmental disabilities and behavioral health needs. The role includes providing consultation to caregivers and community providers, conducting assessments, and developing crisis intervention plans.
