Licensed Practical NurseThe purpose of Forensic Assertive Community Treatment (FACT) is to deliver comprehensive and effective services to individuals (age 18 and up) who are diagnosed with serious mental illness (SMI) and whose needs have not been well met by more traditional service delivery approaches. These individuals also suffer from co-occurring substance use disorders and have histories of criminal legal system involvement. Assertive Community Treatment (ACT) is an evidence-based practice that provides an integrated set of other evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multi-disciplinary mental health treatment team to support recipient recovery through a highly individualized approach that provides recipients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects.Under the direction of the team's Registered Nurse (RN) and/or Prescriber, the Temp Licensed Practical Nurse (LPN) provides medication management and administration service, health education, counseling, and psychoeducation to promote recovery, wellness, and optimal health. The Temp LPN provides at least 80% of services in the community within the framework of core ACT operating principles, professional nursing ethics, and regulatory guidelines.This is a temporary, part-time (17.5 hours/week) position.Shift Hours: This position is part-time, temporary (17.5 hours/week) Monday – Friday from 9:00 AM – 5:00 PM.Salary: The salary for this role is $54 an hour.Location Address: 25 Chapel Street, 11th Floor, Brooklyn, N.Y, 11201. Our office is easily accessible by public transportation.Workplace Flexibility: Flexible - Staff have the flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.What You Will Be Doing:Provides at least 80% of medication management and health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, coordination of consumers' health care services, providing psychoeducation to consumers about health and medications, assessing consumers' mental statusesCompletes and signs progress notes, incident reports, and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulationsCoordinate with FACT RN and prescriber on consumers' medication management needs, health care services, maintaining consumers' medication administration schedulesCoordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providersParticipates in daily team meetings, clinical meetings, and monthly nurses meetings to collaborate and discuss clinical issues and clients' needs with team members and F/ACT nursing staffUse motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering servicesAny other duties as required by Team Leader and senior clinical leadership.What We Are Looking For:Licensed Practical Nurse (LPN) with current New York State licenseAt least three years of experience working with adults (ages 18 and up) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement or limited social supportKnowledge of community treatment, support services, and resourcesHighly organized and self-motivatedProficient in computers.Preferred Skills:Fluency in SpanishOur ValuesAt CASES, we like to move with PURPOSE, which reflects our values:PEOPLE| Hold people's stories with dignityUNITY| Commit to a shared missionRESPECT| Celebrate the strength of diversityPROGRESS| Always work to improveOPTIMIZE| Make the best use of resourcesSOLUTIONS| Work together to solve problemsEMPATHY| Seek to understand othersAlthough we would love to learn about the skills of every candidate, only selected candidates that are selected will receive a response.
