Join a physician-led primary care organization focused on outcomes, not volume, offering a unique opportunity for Primary Care Physicians (MD/DO). This role emphasizes relationship-driven care within a value-based Medicare Advantage model. You will benefit from manageable patient panels, strong care team support, and the ability to practice medicine with time, accountability, and purpose.

Practice Info

Value-based, full-risk Medicare Advantage model
Patient panel capped at ~450 patients
Focus on prevention, care coordination, and behavior change
Dedicated care teams (MA, Care Coordinator, Care Facilitator)
Emphasis on quality, patient experience, and physician wellness

Responsibilities

Provide comprehensive primary care to older adult patients
Maintain full accountability for clinical outcomes and patient experience
Serve as a health coach, guiding lifestyle and long-term health improvement
Coordinate specialty and hospital care while staying actively involved
Lead and collaborate with an interdisciplinary care team
Supervise and remain accountable for Advanced Practice Providers (APPs)
Partner with medical and operational leadership to meet quality and cost goals

Compensation

Salary: $250,000 – $306,714 (based on experience)

Requirements

MD or DO required
Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
Active or eligible state medical license
