Facility:
This community health center is a federally qualified health center dedicated to providing accessible, high-quality primary care services to a diverse patient population. The facility operates Monday through Friday and serves a mix of pediatric and adult patients in a collaborative and supportive clinical environment.

Opportunity:
Advanced Practice - Physician Assistant-Family Practice

Job Details:

Provide comprehensive family practice care to a mixed pediatric and adult patient population
18 patient encounters per shift, Monday through Friday, 8:00 AM - 5:00 PM
Chart closure required within 72 hours
Must have a minimum of 3 years of experience in a similar clinical setting
Active California license required; licensed in CA only
Active DEA required prior to start date
PAs must have a CA Controlled Substance Education Course Certificate
NPs must have a Furnishing License with Schedule II
BLS certification required
Providers must be ORP (order, referring, and prescribing) enrolled in MediCal or willing to enroll during credentialing
Candidate CVs must include month and date on all work experience with all gaps in employment accounted for
Required immunizations prior to start date: Flu Vaccine, TB Status/X-Ray Results (within 90 days of start), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, and Tdap Immunization (within the last 10 years)
Credentialing timeframe is approximately 45-90 days
30-day notice required for ending all assignments
Nearest airport is ACV - Arcata
