Work Environment

This position is entirely onsite, as you will be visiting patients in their homes across the Fords, New Jersey area. Key Responsibilities

• Conduct thorough evaluations of patients in their home environment to assess their physical abilities and rehabilitation needs.
• Develop customized treatment plans utilizing a diverse range of techniques, including therapeutic exercises, manual therapy, and modalities to address pain and improve functional mobility.
• Implement and oversee treatment interventions, documenting patients' progress and adjusting strategies as necessary to meet evolving therapeutic goals.
• Educate patients and their caregivers on self-care, the use of assistive devices, and exercises to encourage independence in daily activities.
• Maintain consistent communication with other healthcare providers and stakeholders to deliver integrated patient care.
• Stay current with industry developments, best practices, and research-backed techniques through continuous professional growth opportunities.

The patients you will serve range from children to the elderly, each facing different challenges due to injuries, surgeries, or chronic conditions. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• Must hold an active and unrestricted Physical Therapist license in New Jersey with no history of sanctions.
• Exceptional knowledge and skill in manual therapy techniques and exercise program implementation.
• Proficiency in using electronic medical records (EMR) systems, particularly Web PT, for accurate patient documentation.
• Demonstrated commitment to ongoing professional development and application of the latest research in physical therapy.

Preferred Experience

• Prior experience in home care settings is preferred, providing insight into the unique challenges and rewards of delivering therapy in a patient's home.
• Experience working with a diverse patient demographic, addressing a variety of physical therapy needs and conditions.

Compensation and Benefits
