Overview
Physical Therapist – Home Health travel assignment in Belle Fourche, South Dakota. Start date 06/22/2026 for a 13-week contract. Weekly pay range $2,121 per week to $2,285 per week. 36 hours guaranteed per week. Shift: Days. Setting: long-term care facility (LTC – Nursing Home). This is a travel temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Perform initial PT evaluations and develop individualized treatment plans for residents in a long-term care setting
- Implement therapeutic exercises, gait training, balance activities, and functional mobility tasks
- Monitor progress, document clinical findings, and adjust care plans accordingly
- Educate residents and families on treatment goals, home exercises, and safety strategies
- Collaborate with an interdisciplinary team to coordinate care and ensure continuity across the care plan
- Ensure documentation meets facility policy, compliance standards, and regulatory requirements
