LVN - Emergency Dept - Per Diem Shift: 6:45am - 7:15pm Candidates must have 1+ years of experience working as an LVN to qualify for this position. Job Description: PATIENT POPULATION:
The patient population served can be all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.
POSITION SUMMARY:
Under general supervision of the Director, Clinical Manager, and R.N., the Licensed Vocational Nurse (L.V.N.) delivers patient care within L.V.N. scope of practice to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient's needs and has an understanding of the range of treatment needed for all patient groups.
Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. To perform this job successfully, an individual must have and maintain current licensure as a Licensed Vocational Nurse in the state of California. Must have and maintain current BLS certification. Must have and maintain Crisis Prevention Intervention (CPI) certification or obtain within one (1) year of assignment to department. Must have IV and blood withdrawal Certification or obtain within 1 year of hire date. If certification is un-obtainable, additional time to obtain may be approved at the CNO discretion. 2 Documents care on the patient care record, accurately, consistently and in a timely manner.
3 * Prioritizes work activities to achieve optimal patient care.
4 * Initiates nursing orders to ensure safety of patients at risk.
5 Completes incident reports; reports incidents to Director, Clinical Manager, or nurse.
6 Ensures availability of supplies and equipment for specific patient needs.
7 Compliance with all regulatory bodies: (DHS).
8 Provides staffing support to the unit as requested by Director, i.e. scheduling flexibility, working extra shifts on short notice, low census days.
9 * Demonstrates ability, flexibility to assist in unusual clinical situations, i.e. temporary reassignment, changes in patient acuity/reassignment, unscheduled admission, and life-threatening patient situations.
10 Provides clinical support to reassigned personnel and prn personnel within L.V.N. practice.
11 * Communicates essential information to Director, co-assigned nurse and/or Registered Nurse. a. Reports significant changes in patient status. b. Provides feedback, positive and constructive, i.e. peer performance,
inter/intradepartmental, etc. 12 * Works within their scope of practice as defined by the California Board of Vocational Nurses and Psychiatric Technicians.
13 Possesses assessment skills to include respiratory status.
14 Administers medication to residents in a safe and competent manner and maintains a medication error rate less than 5%.
15 Practices appropriate infection control techniques.
16 Delegates appropriate tasks to unlicensed personnel and ensures their completion.
17 Interpersonal relationships demonstrate an understanding of personal responsibility to represent nursing and Sierra View Medical Center in a courteous, helpful and productive manner.
18 * Participates in quality assurance activities and integrates findings into practice.
19 * Demonstrates sensitivity and awareness of patient's rights in handling confidential patient information.
20 * "Adheres to hospital standards, policies and procedures relative to situation. a. Fulfills work schedule commitment (attendance). b. Dresses appropriately. c. procedures. b. Demonstrates skill and ability to perform waived testing as per established policy and procedure. c. Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice. d. Engages the patient through teaching in activities that support the NPSGs. 23 Delivers patient care with compassion, empathy and respect utilizing the nursing process model.
24 Assists in obtaining general patient information.
25 * Encourages patient, family, significant other participation in identification of needs, i.e. physical, emotional, educational, discharge, etc.
26 * Under direct supervision of the R.N., identifies pertinent patients' needs/problems.
27 * Is responsible for the treatment of care appropriate to the ages of the patients served, i.e. Adolescent, Adult, and Geriatric. Demonstrates the ability to obtain and interpret information in terms of the patient's needs; possesses knowledge of growth and development; and has an understanding of the range of treatment needed for these patient groups.
28 * Under direct supervision of the R.N. Participates in ongoing revision of individualized patient care plan identifying measurable long/short-term goals and discharge plans.
29 * Initiates physician's verbal/written ordered services.
30 Attempts to ensure patient understanding of special preparation for procedures performed in and by other departments to facilitate a coordinated effort toward optimal patient care.
31 * Delivers basic nursing care utilizing policies, procedures and standards of care.
32 Accurately reports and records assessments, patient data, medication, treatment, procedures and contributes to evaluation of progress towards goals.
33 Evaluates patient response to specific nursing interventions.
34 Assumes responsibility for continued professional growth through participation in continuing education opportunities.
