Young Adult Mental Health Case Manager - Field & Community
Center for Life Management
Center for Life Management is seeking a Young Adult Community Case Manager to support clients aged 15-26 with mental illness and first episode psychosis. You will develop care plans, coordinate benefits, and provide in-home, school and community-based services through a multidisciplinary team. You will document every service, advocate for clients, and ensure a wrap-around approach while maintaining professional conduct and compliance with agency policies. #J-18808-Ljbffr
