Home Care Field Registered Nurse – Otsego County, NY | $85K Location: Otsego County, New York Company: At Home Care Partners Salary: $85,000 per year Position: Full-Time Field Registered Nurse (RN) Build a Rewarding Nursing Career in Home Care At Home Care Partners is seeking a compassionate and experienced Home Care Field Registered Nurse (RN) to support patients and families throughout Otsego County, New York . This full-time opportunity is designed for an RN who enjoys autonomy, one-on-one patient interaction, and providing care within the community. Instead of working in a traditional facility setting, you'll have the opportunity to meet patients where they are and help ensure they receive safe, personalized, and high-quality care at home. As a Field RN , you will conduct nursing assessments, oversee individualized Plans of Care, provide clinical guidance to home health aides, and work closely with patients, families, caregivers, and our internal team. What You'll Do Travel to patient homes and community-based settings throughout Otsego County Conduct comprehensive in-home nursing assessments Perform head-to-toe evaluations and review medications and diagnoses Develop, review, and update individualized Plans of Care (POC) Complete required home health aide supervisions Provide ongoing clinical guidance to aides and caregivers Orient aides to each patient's Plan of Care and individual care needs Monitor patient conditions and identify or report significant changes Communicate effectively with patients, families, caregivers, aides, and office staff Complete accurate and timely clinical documentation Support continuity and quality of care for assigned patients Maintain compliance with applicable healthcare regulations and company standards What We're Looking For Current and active New York State Registered Nurse (RN) license Ability and willingness to travel throughout Otsego County and assigned service areas Medical Professional Liability insurance Strong nursing assessment and critical-thinking skills Excellent written and verbal communication abilities Strong organizational and clinical documentation skills Comfortable using Gmail and Microsoft Word/Excel Ability to work independently in a field-based environment Dependable, professional, compassionate, and patient-focused Must meet all company hiring and onboarding requirements Preferred Experience Home care or home health nursing experience Community health or outpatient nursing experience Conducting in-home nursing assessments Completing aide supervisions Developing, reviewing, or updating Plans of Care Familiarity with Medicaid, Medicare, or MLTC plans Bilingual Spanish/English is a plus Compensation & Benefits Salary: $85,000 per year At Home Care Partners offers: Competitive compensation Weekly direct deposit Health insurance Dental insurance Vision insurance 401(k) plan Transit benefits and dependent care accounts Verizon Wireless and mobile internet discounts Company vehicle and parking access, where applicable Supportive team culture Opportunities for advancement Make Your Nursing Experience Matter At Home Care Partners is a nonprofit 501(c)(3) home care agency committed to delivering compassionate, individualized care to patients in their homes and communities.
