Requirements
- New Hampshire state PT license (valid or pending with ability to obtain by start)
- BLS certification (acceptable AHA/Red Cross copies only)
- Minimum of 1 year of PT experience required

Roles & Responsibilities
- Conduct initial home assessments and develop treatment plans.
- Evaluate patient needs and implement interventions for strength training, balance, and fall-risk reduction.
- Provide education to patients and caregivers regarding home exercise programs and equipment needs.
- Monitor patient progress and adjust treatments as necessary.
- Coordinate discharge to the community when appropriate.

Compliance / Notes
- Orientation: 10 hours
- Flu vaccine required (medical/religious exemptions only)
- Drug screen required within 14 days of start
- Background/license issues must be approved by the facility prior to offer
