Registered Nurse, Home Health

Become a part of our caring community and help us put health first As a Home Health Registered Nurse , you will: Provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager. Confer with physicians to develop and revise the initial plan of treatment based on orders and assessments, under supervision. Coordinate care with healthcare personnel and external providers. Report patient progress to physicians and Clinical Managers regularly. Implement care plans collaboratively with patients and families to address their needs. Supervise and document the work of Home Health Aides and related staff, ensuring quality and compliance. Discharge patients with proper documentation after consultation. Maintain comprehensive medical documentation, including updates, care plans, and communication with other disciplines. Use your skills to make an impact Required Experience/Skills: Diploma, Associate, or Bachelor Degree in Nursing. At least one year of nursing experience preferred, with strong med-surg, ICU, ER, or acute care background. Home health experience is a plus. Valid RN license and CPR certification. Strong organizational and communication skills. Valid driver’s license, auto insurance, and reliable transportation. Pay Range: $55.00 - $76.00 per visit/unit; $80,400 - $110,500 annually, depending on location and experience.
