Your next nursing opportunity starts here. We are seeking a committed Registered Nurse (RN) with proven OASIS experience for a 13-week full-time home health contract. This role requires confidence in delivering skilled patient care, attention to documentation, and the ability to thrive within a structured schedule an ideal fit for RNs passionate about making a meaningful impact through home health services.

Schedule:

13-week contract with strong potential for extension
Full-time, Monday through Friday
On-call responsibilities probable
Set daily visit expectations

Key Responsibilities:

Deliver in-home nursing care and skilled interventions
Complete thorough patient assessments and OASIS documentation accurately and promptly
Collaborate with physicians and interdisciplinary care teams to develop and implement individualized patient care plans
Administer wound care, manage medications, provide disease monitoring, and foster patient education
Consistently communicate patient status updates to the clinical team

Desired Qualifications:

Active RN license (required)
At least 1 year of recent RN experience in home health settings
Demonstrated proficiency with OASIS documentation
Familiarity with HomeCare HomeBase strongly preferred
Dependable transportation and a valid drivers license
Organized, self-motivated, and comfortable managing a full-time clinical visit schedule
