Coordinate daily with nursing staff and support personnel to plan shift services, programs, and activities, ensuring resident care plans are fully executed. Prepare and administer medications; review medication cards for completeness and accuracy. Review resident care plans for appropriate goals and revisions based on nursing needs. Plan, organize, develop, implement, and interpret programs and policies that provide quality care. Provide leadership to nursing personnel; conduct daily unit rounds to confirm staff compliance with resident care plans. Develop and maintain rapport with nursing staff, other department personnel, residents, family members, and all individuals involved in resident care. Participate in facility surveys by authorized government agencies. Qualifications Licensed Practical Nurse (LPN) degree from an accredited college or university. Current, unencumbered, active license to practice as an LPN in Oregon. Knowledge of nursing and medical practices, laws, regulations, and guidelines applicable to nursing care facilities. Chart nurses’ notes in an informative, descriptive manner that reflects care provided and resident response. Experience with electronic medical records and computer documentation systems. Maintain confidentiality of all resident care information in accordance with HIPAA guidelines. Fluent in reading, writing, and speaking English. Active CPR/BLS Certification required.
